Eligibility
Clients of any age with a diagnosis of chronic mental illness, as defined in the latest edition of the DSM, who have been assessed and determined to require Mental Health Targeted Case Management (MHTCM) services.
Mental Health Targeted Case Management
Targeted Case Management (TCM) services are designed to support clients within specific populations. Under the Texas Medicaid Program, Mental Health Targeted Case Management (MHTCM) is available to clients of any age who have a single chronic mental illness or a combination of chronic mental illnesses, as defined by the latest edition of the American Psychiatric Association’s (APA) Diagnostic and Statistical Manual of Mental Disorders (DSM).
Clients must be assessed through a standardized evaluation process and determined to need MHTCM services. These services help clients access essential medical, social, behavioral, educational, and other community supports to improve overall functioning and well-being.
Types of Case Management

Intensive Case Management services are primarily community-based and are provided directly to the client or to their legally authorized representative (LAR) on the client’s behalf. These services help clients and their caregivers or LARs obtain, coordinate, and access the care and resources necessary to meet the client’s individual needs.

Routine Case Management services are primarily office-based and assist clients, caregivers, or legally authorized representatives (LARs) in obtaining, coordinating, and accessing the care and services necessary to meet the client’s individual needs.
